If you‘re running a nonprofit, I know how important finding sponsors can be to the success of your mission. But at the same time, you’d probably rather focus on actually running your organization instead of getting distracted by all the tasks required to find sponsors successfully.

Below, I'll share some ready-to-use sponsorship form templates (for online or in-person), some of the best tools to build online sponsorship forms, and some real-life examples of sponsorship pages from different organizations to give you some inspiration.

What is a sponsorship form?

A sponsorship form is a physical or digital form that people or organizations can fill out to request or provide sponsorship to an organization.

For example, if you run a non-profit organization that hosts events for the local community, a sponsorship form would help you share information and collect details from corporations or individuals who want to financially sponsor your event.

Ideally, your sponsorship form will outline the agreement between both parties and detail what the sponsor receives in return for its contribution.

When I share real-world examples below, you'll see that there can be a lot of variation in what a sponsorship form or sponsorship page looks like.

Some are just basic forms that collect contact details from potential sponsors, while others outline the different sponsorship opportunities that are available in depth.

Regardless of type, a sponsorship form aims to establish a long-term relationship between the sponsor and your business. This intention differentiates sponsorships from donations, too. While donations are typically one-time gifts, sponsorships cultivate ongoing support, which is why I've noticed it is especially impactful for non-profits.

Developing a sponsorship form with limited resources can be tough. Sponsorship form templates can help. (Don‘t worry — I’ve compiled a list you can reference.)

Pro tip: Include a call to action (CTA) on your website inviting visitors to become sponsors.

Sponsorship Form Template Components

While you should feel free to customize your sponsorship form to your organization, I do think there are some basic building blocks that all quality sponsorship forms should include.

In my experience, an effective sponsorship form should accomplish the following goals:

  • Collect the sponsor names.
  • Keep track of sponsors' addresses, which can be especially important for reporting purposes.
  • Collect other important contact information, such as emails and phone numbers.
  • Track the amount of money the sponsor is willing to commit.
  • Provide the sponsor with any relevant details, such as the different sponsorship levels that are available (if you have preset tiers).

To start, these details are essential for tracking sponsorship amounts and being able to communicate with sponsors. Beyond that, you also might need this information for more official record-keeping and reporting.

Your organization might certainly have its own unique needs that go beyond these components, but starting with these fundamental building blocks will help you achieve success.

The actual design of your form can also be an essential component. While creative designs can be fun, you should never sacrifice user experience and the usability of your form should always come first. Potential sponsors must find your form easy to navigate and complete.

Pro tip: Try to anticipate any questions potential sponsors may have, as this can help you address them early on and avoid sponsors having any doubts. For instance, it’s worth including information about your sponsorship goals. You can also briefly describe your mission and monetary needs. If you’re taking sponsorship requests, clearly outline what you’re looking for in potential candidates.

Why use a sponsorship form template?

Having a sponsorship form is important to help your organization secure new sponsors. While there‘s nothing wrong with choosing to create this form yourself, using a pre-made template can help you save time while also ensuring that your form isn’t missing any critical details.

Pro tip: Even if you use a template, you can still fully customize it to make it unique to your organization. Basically, I don't think you need to worry about looking “formulaic” because you can still tweak the template to make it your own.

Excellent Sponsorship Form Template Samples

To help you get started and give you some inspiration, I've collected a few of my favorite sponsorship form templates below.

Again, I recommend using these as a starting point for your own form — you definitely don't have to use these examples “as is.”

Basic Sponsorship Form

This is one of the simplest sponsorship form examples that I could find, but sometimes things are simple for a reason. This form works great to collect all of the essential information that you need from sponsors. Its sections are digestible and offer a succinct description of what the sponsorship entails.

What I like: This form is simple and straightforward and you can easily modify it to work with pretty much any type of organization. Because this template is designed to be printed out, I think it works best for in-person events where sponsors can write their own information (such as a charity fundraising event).

Online Sponsorship Form

Note — the example screenshot above shows the “unstyled” form. While the form fields and layout are good, I recommend styling the form to match your website or organization.

If you have a website, you can leverage it to connect with sponsors online. By going beyond in-person forms like the previous example, you can access a much broader pool of sponsors and let sponsors make their commitments from anywhere with a computer.

What I like: Because this template is based on the Wufoo form builder, you can easily customize every field and layout option to fit your organization. You could also use it as a jumping-off point for a form that you create yourself. Later in this post, I'll share some of my favorite sponsorship form builder tools.

Tiered Sponsorship Form

This letter describes different sponsorship “tiers” available to supporters while communicating the benefits of sponsoring. I think that providing multiple sponsorship tiers like this is a great way to let sponsors make an impact regardless of the amount of money that they're willing to commit.

What I like: While this sponsorship form example has a lot more explanatory text than the previous examples, I think that having this level of information can be great when explaining the benefits that sponsors will receive in exchange for their support.

Event Sponsorship Form

If you're trying to find sponsors for a specific event, this form is a great example of how you might do it. It includes details about the intended purpose and how sponsors can contribute.

What I like: Like the previous example, this form does a great job of outlining what sponsors get for their monetary contributions. It's also available in the DOCX format, which means that you can easily customize it for your organization using Microsoft Word or Google Docs.

How to Create an Online Sponsorship Form

I’ve shown you what an engaging sponsorship form template looks like. Now, it’s time to create your own. Don’t worry — I’ll help you through this process, too, so you don’t have to do it alone.

1. Log into your form-building software or account.

Since HubSpot provides a free form builder, I’ll walk you through the process using that. If you don’t have a HubSpot account, create one for free.

Once you log in, go to your form-building interface. In HubSpot, the “Marketing” tab is where you can build forms and customize ads, emails, and web pages.

2. Select a form type.

Most form builders provide you with the opportunity to select a form type. Here’s where you can decide whether to put the form on a new, standalone page or embed it into an existing one. You can also create a pop-up form to grab a visitor’s attention while they’re scrolling through your site.

3. Add form fields.

Next, choose the contact fields you’d like to add. Our example form comes with a standard email field, but consider adding first and last name, street address, and phone number for these purposes.

4. Include form fields that collect sponsorship information.

Aside from basic information, you’ll also need to include fields that obtain specific information for sponsorships. These are fields like donation amount, company information, and checkboxes for contract compliance.

In HubSpot, scroll down the left-hand side menu to the “Create New Field” section. Here, you can select the style of field you want and define the data you want to capture.

5. Create a follow-up email.

After a sponsor completes your form, send them an automated message to thank them for their commitment. You can also use this email to provide them with additional information. The best way to do this is through a follow-up email triggered automatically upon form submission.

6. Publish your sponsorship form.

Congratulations — you’ve officially created a sponsorship form! Now, you can copy and embed the form code into your website or get a shareable link that sends the form to specific contacts in your CRM. That way, if you have sponsors that you know are interested in partnering with you, you won’t have to create a brand new web page to collect their information.

Pro tip: If your form aesthetic doesn’t match your branding, it can erode trust. If you publish your form on a customer-facing web page, the design must be cohesive with the look of your website. (Psst: I’ve listed some tools below to help you accomplish this.)

What are the best sponsorship form tools?

To simplify things while also gaining access to more functionality, I recommend using a form builder tool to create your sponsorship form instead of trying to do it manually.

These tools let you set up your form using simple drag-and-drop interfaces while also making it easier to sync sponsor information to other tools that you're using, such as the free HubSpot CRM.

Based on my own experience, here are some of my favorite tools for creating any type of form, including sponsorship forms.

1. HubSpot

Thanks to its drag-and-drop form builder, HubSpot makes it super easy to create a sponsorship form that you can embed anywhere on your website — or even display as a pop-up if that better suits your needs.

You can construct your form using a variety of different field types, including text fields as well as more structured field types such as date pickers. You can also fully customize the style and layout of your form using a visual editor, which I like because it means you can create a custom sponsorship form without needing any special technical knowledge.

One of the best parts about it, though, is that you can automatically sync all of your information to the free HubSpot customer relationship manager (CRM), which lets you easily keep track of all your sponsors' contact details and connect with them via different channels.

Gravity Forms is a WordPress form plugin that makes a great solution for creating a sponsorship form. To start, it offers a drag-and-drop form builder that makes it easy to set up a form that's customized to your organization.

Where I think it really stands out is its ability to accept sponsorship payments directly from your form and also its ability to integrate with a variety of third-party services.

Gravity Forms offers a direct integration with the HubSpot CRM, along with many other services. If you can't find a direct integration, it also works with Zapier.

3. Forminator

Price: Free to start. Pro plans from $15 per month.

Forminator is a free WordPress plugin that also includes everything that you need to create a sponsorship form for your organization.

In addition to a non-technical form builder interface, it also streamlines sponsorship payments with support for Stripe and PayPal.

Beyond its payment support, it also integrates directly with other services such as the HubSpot CRM, which means that you can easily sync sponsor information directly to HubSpot.

4. HubSpot WP Plugin

Above, I talked about some of the many ways in which HubSpot can help you create a great sponsorship form and manage your sponsor relationships.

If you're using WordPress, the HubSpot WordPress plugin lets you access a lot of that functionality without needing to leave your WordPress dashboard, which can streamline a lot of sponsor admin tasks for your organization.

From WordPress, you'll be able to create sponsorship forms (and other form types) and also view sponsor details from the synced HubSpot CRM.

5. WPForms

Price: Free to start. Paid plans from $99 per year.

WPForms is another WordPress form builder plugin that can make a great solution for sponsorship forms.

You can get started from a blank slate or use the pre-made sponsorship form template. Regardless of where you start, you can customize everything using a drag-and-drop form builder interface.

The premium version also includes more advanced functionality, such as the ability to process sponsorship payments directly from your form and sync sponsor details to the HubSpot CRM.

6. Wufoo

Price: Free to start. Paid plans from $16.25 per month.

Wufoo is an online form builder that lets you embed your sponsorship form anywhere. You can add it to any website-building platform or even share a direct link to the sponsorship form, which I think could work great if you want to share it over email or social media.

You can design your form using a non-technical builder and also integrate your form with hundreds of other tools, including the HubSpot CRM.

I also found that Wufoo comes with a pre-built sponsorship form template, which I covered earlier in this post. I think it offers a great way to get started quickly.

The Best Sponsorship Form Templates

Some of the tools that I covered above already include pre-made templates for sponsorship forms that you can use within the builder.

However, if you‘re looking for some additional templates, I’ve collected some of the best sponsorship form templates that you can use for your organization.

1. JotForm

Jotform is an online form builder tool that includes a large collection of over 42 different sponsorship form templates, along with hundreds of templates for other use cases, too.

The sponsorship templates cover a range of different use cases, including general sponsorship inquiries as well as event sponsorships, corporate sponsorships, and much more.

In my opinion, some of the templates are a little vague and only have a few questions, while others are quite detailed and collect a lot of relevant information. You can choose the approach that works best for your organization.

Regardless of which template you choose, you‘ll be able to fully customize it to match your organization’s needs.

What I like: Jotform offers a really broad selection of templates. Whether you just want a generic starting point or you're looking for a specific type of sponsorship, you can probably find a good template in its collection.

2. Sample Templates

Sample Templates provides specific sponsorship forms for charity support, immigration sponsorship, and sports fundraising.

Rather than offering a web-based form builder like many other template sources, these templates are available in PDF and DOCX formats (though I noticed that not all templates are available in both formats).

What I like: These templates can be a great option if your organization still relies on paper forms. For example, if you want to let people sign up in person at an event that you're hosting, these templates could be a good option.

However, if you want to add a sponsorship form template to your website, you'll want to use a different solution (though I think that you can still use these templates as inspiration for whichever form builder tool you choose).

3. Words Templates

Words Templates can be another good option if you're looking for sponsorship form templates for Microsoft Word or Google Docs.

All of the sponsorship form templates at Words Templates are in the DOCX format, which means you can open and edit them in any word processing software, including Microsoft Word and Google Docs.

Once you're finished tweaking the templates, you can print them and use them in person.

What I like: I think that Words Templates can be another good option to consider if you're primarily searching for templates that you can use for a physical sponsorship form that you use at in-person events.

4. HubSpot Resource Library

Sponsorship Page Examples

Sponsorship pages are just as important as the sponsorship forms.

The difference between the two is that forms capture your potential sponsor’s information, while the page gives you the opportunity to tell the sponsor why they should invest in you. Many sponsorship pages also include a form directly in the page (though this isn't always the case, in my experience).

Below, I've collected five great examples of sponsorship pages to give you some inspiration for your own.

1. National Public Media

With over 43 million weekly listeners (including my mother), National Public Media (NPM) supports National Public Radio (NPR) and other public radio programs.

Beyond average listeners, NPR’s sponsor page knocks it out of the park. The organization offers an in-depth look at its listeners. I like that sponsors have access to key information about their members, such as testimonials, level of influence, and skill set.

The organization also reassures sponsors that NPM can help them reach their target audiences by providing lots of useful data and facts.

What I like: NPM does an amazing job of providing specific, data-driven benefits that sponsors can achieve. The data covers a range of relevant categories (scroll down the page to see most of it), and I found it to be very persuasive. If you have this type of data, I definitely recommend including it on your organization's sponsorship page.

NPM also has a separate “Podcast Sponsors” page, which I think is another great way to provide value to sponsors.

2. WordCamp Europe

WordCamp Europe is a yearly gathering of the WordPress community in different locations around Europe. It's managed and run by the WordPress Foundation, a registered non-profit.

The WordCamp Europe 2025 sponsorship page is one of the most detailed examples on this list, packed with different information for sponsors.

It covers the benefits to sponsors, the different sponsor packages that are available, and more. It also has a sponsorship form at the bottom, which can give you further inspiration for when you create your own sponsorship form.

What I like: This page does a great job of communicating the benefits that sponsors receive in exchange for their sponsorship. If you're creating your own sponsorship page, I think that clearly highlighting benefits like this is a great way to convince sponsors to make a pledge.

If you have different sponsorship tiers, you should also cover that information in detail on your page, which I think WordCamp Europe also does a great job of doing.

3. Destination: Home

Destination: Home is a Silicon Valley-based advocacy group focused on ending homelessness in the Bay Area. This group partners with corporate donors, community groups, and nonprofits to advocate for policies designed to prevent homelessness.

What makes this sponsorship page unique is that it isn‘t about Destination: Home seeking out its own sponsors. Rather, it’s a page where non-profit organizations working on similar causes can apply to receive sponsorship from Destination: Home.

What I like: This page is a good reminder that a sponsorship form can work in both directions. If your organization provides sponsorship to other organizations or individuals, I think this is a great example of how you might want to do it.

4. KEXP

KEXP is a non-profit, listener-supported radio station based in Seattle, WA. KEXP also runs a popular YouTube channel that I watch a lot, which is how this example got on my radar.

To help cover its cost, KEXP accepts sponsors in addition to donations from individuals.

The KEXP sponsorship page is divided into two columns and includes everything that you need. On the left side, KEXP includes some benefits and facts about sponsorship. Then, on the right side, KEXP embeds the actual sponsorship form that sponsors can use to get more information.

What I like: I think KEXP's use of hard facts is really effective for convincing people to sponsor them. For example, the fact that 80% of public radio listeners prefer to buy products from companies that support public radio is very persuasive.

I also like the simplicity of the two-column design. All the content fits above the fold (on my monitor, at least) but I still think they managed to get a ton of important information into the page.

5. Philadelphia Museum of Art

While some visitors to Philadelphia might only interact with the Philadelphia Museum of Art via its famous Rocky Balboa statue, it's also packed with more than 240,000 objects and art pieces spanning more than 2,000 years of humanity.

To help cover the costs of running such a large organization, the Philadelphia Museum of Art sponsorship page outlines the three primary ways that sponsors can get involved: exhibitions, educational programs, and annual events.

Further down, the page lists some of the more structured corporate partner sponsorships, which range in price from $1,500 to $25,000.

However, one thing that I think could improve this page would be to include an actual sponsorship form directly in the page, rather than just asking people to send an email to learn more about sponsorship opportunities.

A sponsorship form can save time for both parties by providing more structure to the interaction.

What I like: I think the Philadelphia Museum of Art has done a great job of clearly outlining what types of sponsorship opportunities are available. By clearly highlighting these details up front, sponsors can know exactly how they can get involved, rather than being forced to guess.

Use seamless sponsorship form templates to attract sponsors.

Your organization needs a sponsorship form if you want to connect with sponsors effectively. However, I don't think that means that you need to spend a bunch of time on it.

By using a non-technical tool and some of these pre-made sponsorship form templates, you can get up and running with a great-looking and effective sponsorship form in very little time.

I recommend using the examples above as inspiration and then creating your form using one of the templates and/or tools above. Then, you can get back to focusing on the meaningful work that your organization performs.
